Amphotericin B
Combining irinotecan with amphotericin B could increase your risk for amphotericin B side effects, such as kidney damage, low blood pressure (hypotension), and breathing problems. Your healthcare provider may choose to monitor you more closely if you use these medicines together.
 
Biologic and Immunologic Medications
Combining irinotecan with biologic and immunologic medications could increase your risk for potentially serious side effects, including low blood cell counts and infection. In addition, people who receive irinotecan along with bevacizumab appear to have an increased risk for diarrhea and low white blood cell counts.
 
Clozapine
Taking clozapine and irinotecan together may increase the risk of dangerously low white blood cells, which would increase your risk for potentially serious infections. Talk to your healthcare provider before taking clozapine during irinotecan treatment. However, do not stop taking clozapine unless your healthcare provider specifically tells you to do so.
 
CYP 3A4 Inducer Medications
The liver breaks down irinotecan using enzymes known as CYP 3A4 enzymes. Medications known as CYP 3A4 inducers can increase the activity of these enzymes. Combining irinotecan with one of these CYP 3A4 inducers may decrease the level of irinotecan in your body, potentially making it less effective. Your healthcare provider may recommend you stop taking certain CYP 3A4 inducers before beginning irinotecan treatment.
 
CYP 3A4 Inhibitor Medications
The liver breaks down irinotecan using enzymes known as CYP 3A4 enzymes. Medications known as CYP 3A4 inhibitors can decrease the activity of these enzymes. Combining irinotecan with one of these CYP 3A4 inhibitors may increase the level of irinotecan in your body, potentially increasing your risk for side effects. Your healthcare provider may decrease your irinotecan dose if you experience serious side effects.
